The Meyer Optik Gorlitz Orestegor 200mm f4 (and its close brother, the Pentacon 200mm f4), is often nicknamed the "king of bokeh" due to its 15 blade aperture and long reach. This combination creates a nice soft bokeh and good background separation.
Despite being a bit larger than some more recent manual 200mm lenses, it is not heavy and balances well on modern cameras.
